Louisiana DMV Information: Changing Your Name and Address

In Louisiana, if you are looking to change your name, car registration, or address, the Office of Motor Vehicles (OMV)) makes it a simple and easy process. Your local LA OMV must have your most updated personal info on file.  To correct this information, simply complete and submit an Application for Change of Address or Request for Vehicle Record Information form. Additionally, you may need to provide proof of your new address.

Changing Your Address at the Louisiana OMV

Whether you are new to Louisiana or just moving within Louisiana, you must notify the OMV. It is important to remember to change your address within ten days of moving. You can only do so in in-person, and please remember to bring the following:

  • Current Louisiana driver’s license/ID card
  • Proof of insurance
  • Payment for any fees (between $13-17)
  • Your Social Security number

It is important to note that you CANNOT change your address by mail or online.

Changing Your Car Registration at the Louisiana OMV

If you need to update your vehicle registration, it is essential to go either in person or do so by mail. You will need to provide the following:

  •  Certificate of registration

Change Name with the SSA in Louisiana

If you need to change your name for any reason, please be sure to notify the Social Security Administration first and then the GA DDS. Changing your name can only be done in person or by mail. You cannot go online and change your name.

You must provide this information:

  • Completed Form SS-5, Application for a Social Security Card
  • Certified name change documents such as:
  • Proof of identity such as:
  • Proof of citizenship such as:
    • Birth certificate
    • Certificate of citizenship
    • Certificate of naturalization

All the documents you provide must be either original or certified copies. Your SSN will not change after completing your name change.

Change Name with the Louisiana OMV

After changing your name legally and obtaining the document with your name change from the SSA, you can visit the nearest LA OMV location within 60 days. This portion can only be one in person. At your visit, you must provide the following:

  •         Evidence of name change such as:

o   Birth certificate

o   Marriage license

o   Adoption papers

  •         Your driver’s license or ID
  •         Payment of $13-17, depending on your location
