West Virginia Duplicate Title: How to Request One

To begin:

Visit a local West Virginia Division of Vehicles (DMV) office/location.

You’ll need to file the Affidavit of a Duplicate title for a Vehicle or Watercraft (DMV-4-TR) and pay a $15 fee to receive your duplicate West Virginia title. All’s needed is your personal and vehicle information when filing the form.

Replacing a title in-person requires you to:

  1. Locate your local WV DMV
  2. Show proof of ID
  3. Show proof of vehicle ownership
  4. Submit your form DMV-4-TR
  5. Pay the application fee

Use this time to update your personal information at the DMV.

Updating your information keeps your records up-to-date and stops any hassles when requesting documents or proving items later. This is good practice if you find yourself moving frequently.
