Missouri offers different record types:
- Without personal information – Documents share traffic and driving-related information

Fees for the driving history report includes:
- Driving record: $2.82
- If faxed: Add $0.50
- If processed in-person: $2

Driver Record Request in Person, By Mail, Fax, or Email
The driver record request is essentially the same process minus how it’s submitted.
- Download and fill out the Request from Record Holder (DOR-4681) form
- Sign and notarize the form
- Pay for the records or include a check or money order
- Process the forms to a DOR office, mail, email, or fax them

Requesting Records of Others
You may request other drivers’ records given you’re doing so as a govt agent, verifying information for criminal/civil legal actions, are an insurance agent, or doing a background check. You’ll include the driver’s name, license number, and date of birth when processing the request.
You may need to file a Request for Social Access Code (DOR-4678) if requesting driver documents including their personal information. This gives an access code, once approved, letting you access the driver’s information.
