How to Change Your Personal Information at a Michigan DMV
You must contact the Michigan Secretary of State (SOS) to report a change in name or address. The change of name and address are important for identification and mailing purposes. The change name will provide your correct identification to get a driver’s license or state ID to vote. Also, you must receive all your mail at your current address.
Change Name at the Social Security Administration (SSA)
The SSA is the first place you must report your change name. It will update your file and link your new name to your current SSN. Once the change name is official, you can go to the Michigan SOS to update your current Michigan driver’s license or state ID.
You must provide the SSA with these documents to have your change name:
- Your current driver’s license or state ID (the next document will prove you change the name)
- Proof of your name change:
- Marriage license
- Divorce Decree
- Court document
- Pay fee

This can be done while you are at the SOS office. To complete a car registration and title change, you must:

- Your title certificate

To change your address at the Michigan SOS, you have three ways to do so. You can apply for an address change by mail, online, or in person.

Complete Michigan Change of Address/Voter Registration Form (Form ED-109W). Provide the necessary information and forward the document to:

This is the fastest way to do an address change. Go to the Michigan SOS website and provide the information required. Once you complete the online process, you will receive a letter with your new information.
In Person
You can visit a Michigan SOS office and complete the change of address form. If you did not renew either, you will then be given a sticker to place on your current driver’s license or state ID.
