How to Change Your Personal Information at a Florida DMV

If you reside in Florida and you just had a name change, there are some departments within the government that just must notify you. Do you have a Florida driving license or a state-issued ID? If so, you must reapply for a new ID with your name change. Also, you must change your car title and registration if you own a car. You must also notify the voter’s registration office and lastly the Social Security Administration office of your name change.

Notifying the Florida Department of Highway Safety and Motor Vehicle of the Name Change

Once you had a name change, you have up to 10 days to get a new driver’s license or state ID card. At the Florida DHSMVC, a name change can only be done in person. To proceed with the process, you must provide the original or certified copies of:

  • Current Florida driver’s license or state ID
  • Proof of citizenship such as:
    • Birth certificate
    • Passport
    • Certificate of Naturalization
  • Proof of name change such as:
    • Marriage certificate
    • Divorce Decree
    • Court documents
  • Social Security Card
  • At least two documents that show proof of Florida residency
    • A current utility bill
    • Bank statement
  • Fee

Notifying the Social Security Administration of Name Change

When you notify the Social Security Administration office, it can be done by either mail or in person. To proceed with informing the SSA, you must provide your original or certified copies as required. The list below will provide all the necessary documents that must accompany the name change request. Here is what you must submit:

  • Completed Application for a Social Security Card (Form SS-5)
  • Name change documents such as:
    • Marriage certificate
    • Divorce Decree
    • Court documents
  • Proof of identity like:
    • Driver’s license
    • Passport
    • State ID card
  • Proof of citizenship such as:
    • Birth certificate
    • Passport
    • Certificate of Naturalization or Citizenship
    • Proof of residence such as:
    • Voter registration card
    • Certificate of citizenship
    • Certificate of Naturalization

Florida Name Change for Car Title and Registration

Once you have completed the name change with the Florida DHCMV you can apply for a replacement title and registration card. To do so, you must:

  • Submit your title with the new name
  • Submit the signed title
  • Pay the cost of the title and registration card

Name Change at the Florida Voter Registration

Informing the Florida Supervisor of Elections in the county about your name change can be done in person, by phone, or by mail.

Your visit to the Florida DHSMV would have given you the chance of informing the Supervisor of Elections about your name change. A form is provided to register or make any changes to your voter registration form.

In most cases, it is best to inform the Social Security Administration Department first of a name change than the other departments. In Florida State, this is also advisable to do as well.

Once the SSA receives the form, they will send an update to the Florida DHSMV.
